Position Title: Licensed Practical Nurse (LPN) Long Term Care (LTC)
Location: Colby KS 67701
Employment Type: Contract
Shift: Night Shift
Schedule: 12-Hour Shifts Variable Nights

Job Summary:
Provide compassionate nursing care to long-term care residents during night shifts including medication administration resident monitoring and support of daily clinical operations in a skilled nursing environment.

Requirements:

• Active Licensed Practical Nurse (LPN) license required
• Long Term Care (LTC) or skilled nursing experience preferred
• Strong medication administration and clinical documentation skills
• Ability to work night shifts and adapt to facility needs
• Strong communication and teamwork skills
• Ability to work independently in a long-term care setting
• No scheduling restrictions required

Responsibilities:

• Provide direct nursing care to LTC residents per care plans and physician orders
• Administer medications and treatments safely and accurately
• Monitor resident conditions and report changes to RN/physician
• Maintain accurate documentation and charting
• Assist with admissions discharges and resident assessments
• Ensure compliance with infection control and facility policies
• Collaborate with interdisciplinary healthcare teams
• Promote resident safety dignity and comfort during night shifts

Facility Notes:

• Hospital-based facility with attached LTC unit
• No overtime beyond 37.5 hours per week allowed
• No travel partners permitted
