This posting is a joint competition between the Referred Care Clinic and the Centre de santé Constellation Health Centre (CSCHC).
It will be used to fill1permanent full-time and 1 term full-time position both working 75 hours bi-weekly. It will establish a 12-month eligibility list which may be used to fill similar positions including permanent term AOC and casual for Whitehorse based Community and Primary Care positions.

Who we are
Community and Primary Care (CPC) Nurse Practitioners (NPs) report to various CPC managers and may work in any of the following CPC Clinics or programs (as required):
- Referred Care Clinic (RCC) / Opioid Treatment Services
- Centre de santé Constellation Health Centre (CSCHC)
- Whitehorse Walk-in Clinic (WWIC)
- Withdrawal Management Services
- Yukon Healthy Living Program (YHLP)
Your role at the Referred Care Clinic (RCC)
The Referred Care Clinic is a primary care clinic for vulnerable adults with unstable mental health and/or substance use disorders. The RCC also operates alongside Opioid Treatment Services which supports any resident wishing to seek help in managing their opioid use. The RCC is a multidisciplinary team composed of outreach support workers outreach licensed practical nurses registered nurses nurse practitioners physicians social workers and medical office assistants. The referred care clinic is embedded within the Mental Wellness and Substance Use Services (MWSU) branch of government and we work closely and collaboratively with all of MWSU including direct support in the Withdrawal Management program.
As a Nurse Practitioner you will be part of multidisciplinary team that is dedicated to providing high quality person-centred and trauma informed care to people seeking primary care and opioid treatment from Mental Wellness and Substance Use Services. You will use your clinical expertise and understanding of theory to provide advanced assessments that will help guide treatment and ideally result in improved health-related outcomes for the individual. This role also supports Opioid Treatment Services and would support professional training to the successful incumbent to provide opioid agonist therapy when indicated.
Your role at the Centre de santé Constellation Health Centre (CSCHC)
The Centre de santé Constellation Health Centre is a is a bilingual primary health care clinic providing high quality primary care to its paneled clients utilizing an interdisciplinary NP provides primary health care in close collaboration with other medical and allied health staff. The NP works closely with health and social care providers located within Whitehorse and throughout the Territory. This includes physicians nurses medical office assistants as well as other social and allied health professionals.
This position liaises with key partners to provide holistic accessible and equitable care for all Integrated Health Services clients and contributes to an environment that is inclusive non-discriminatory and physically psychologically and culturally safe.
NPs are advanced practice primary health care providers who blend research and their specialized nursing knowledge and experience in complex decision-making with their legislated authority to practice autonomously to meet the health needs of individuals families and communities. NPs provide direct clinical comprehensive care or may be consulted for advice for care and are authorized to diagnose diseases disorders injuries or conditions; supply prescribe administer and/or monitor the effectiveness of drugs and treatments; order and interpret lab and diagnostic tests; and perform procedures within their legislated scope of practice.

Essential qualifications

- Registered or eligible to be registered as a Nurse Practitioner in good standing with the Yukon Registered Nurses Association
- Recent experience providing primary care as a licensed Nurse practitioner
- Experience working within a primary health care team in a collaborative practice model is considered an asset.
- Experience working with either perinatal clients clients accessing gender affirming care clients requiring ongoing chronic disease management and clients requiring management for mental wellness and/or substance use challenges would be an asset
- Demonstrated Northern remote or rural experience providing primary health care is an asset

- Proficiency and comfort working with electronic medical records and other digital systems including computerbased charting and documentation.
- Ability to assess and manage both acute health concerns and chronic disease in a primary care setting

Conditions of employment

- Current Nurse Practitioner registration in good standing with the Yukon Registered Nurses Association
- RCMP Security Clearance with Vulnerable Sector
- TB screening
- CPR Level HCP
- Valid Class 5 Drivers Licence or equivalent
Job requirements
This job requires you to perform and/or be exposed to the following:
- Potential of Exposure to upset and verbally/physically abusive clients
- Exposure to infectious materials (ex. Bodily fluids)
- Periodical travel outside Yukon for conferences meetings or other forms of professional development
- Occasional travel for patient home visits or pop up clinics at other community locations
- Regular and extended use of computers for electronic medical record (EMR) documentation clinical information systems and other digital programs required for service delivery.
- May be required to participate in an afterhours schedule to support clinic services outside of regular business hours.
- The successful candidate(s) may be required to use a fleet vehicle to pick up supplies or make deliveries on behalf of the clinic or branch
