JOB SUMMARY:
- Supervise coach and support Direct Support Professionals to ensure consistent highquality personcentered services.
- Provide ongoing training and mentoring to DSPs in communication behavior support and daily program practices.
- Monitor DSP performance through regular observations feedback sessions and documented evaluations.
- Train new staff in the required documentation for the programs services and goals of the individuals served
- Create and manage staff schedules assignments and daily coverage to maintain required ratios and smooth program operations.
- Ensure DSPs accurately complete documentation including daily notes incident reports and progress tracking.
- Inform the Day Program Manager of any issues that arise.
- Complete all required yearly training by December of each year.
- Screen referrals and submit them to the Day Program Manager for approval and coordinate admissions
Consult with referring agencies care providers and residential social workers regarding individual progress needs and service coordination. - Develop service plans for each individual using information from individual and team observations and input and through the completion of assessment materials.
- Ensure service plans are completed on time and are an accurate representation of the individuals learning and assistance needs
- Initiate and coordinate individual service plan meetings as needed
- Report on individual progress needs and behaviors at Annual and Quarterly meetings as scheduled
- Intervene and guide DSPs as individual emergencies arise
- Provide mentoring services (training and personal care) with individuals as needed
- Meet all paperwork reporting and deadline requirements
- Complete: Release of Information forms Human Rights Forms Emergency Medical Forms
- Complete discharge summaries as required. Forward charts to be closed to the Quality Improvement within 30 days of discharge
- Enter billing in extended by Wednesday weekly.
- Review DSP progress notes and observe individual/staff interactions in order to teach DSPs how to teach skills to individuals effectively.
- Complete Annual Reviews Quarterly Reviews and progress notes and data sheets after assessing the individuals status and forward them to the referring agency.
- Audit individual charts periodically for completeness and ensure required annual forms are updated and filed.
- File and mail quarterly reports as required.
- Assist the Finance Department in tracking and resolving billing problems
- Lead team meetings and communicate program updates expectations and policy changes clearly to DSP staff.
- Address staff performance deficiencies and workplace conflicts in a timely and professional manner implementing corrective actions in accordance with organizational policies and procedures
- Conduct and document monthly Day Support Program smoke alarm and fire drill tests
- Monitor program supplies and equipment needs ensuring timely ordering and inventory management to support daily operations
- Office and records administration techniques and procedures
- The structure and content of the English language including the meaning and spelling of words rules of composition and grammar
- Microsoft Office Word Outlook SharePoint Teams and virtual meeting applications such as Zoom Webex Google Meet
- DePauls policies procedures and standard practices and the rules and guidelines established by regulatory and governing agencies such as DBHDS and DMAS
- Relevant policies procedures and strategies to promote the safety and protection of people data and property
- Methodologies and resources used to assess the needs of the listener or audience to share information or engage in ongoing communication
- Give full attention to what other people are saying taking time to understand the points being made asking questions as appropriate and not interrupting at inappropriate times
- Verbal and written communication and body language that conveys information ideas or inquiries effectively and concisely that purveys professional composure emotional intelligence thoughtfulness patience and respect amidst change ambiguity and within the daily trajectories of work
- Use standard office equipment such as telephone calculator printer scanner cellular phone tablet computer shredder
- Use logic and reasoning to identify the strengths and weaknesses of alternative solutions conclusions or approaches to problems
- Collect and analyze data
- Develop and maintain professional working relationships and appropriate boundaries with individuals served coworkers community partners and business associates
- Work collaboratively within a team environment and build strategic relations with community partners coworkers management and business associates
- Communicate effectively in both oral and written form
- Make arithmetical computations
- Operate a personal computer with a general understanding of MS Office Suites virtual meeting software the Internet and department or program-specific software
- Observe and evaluate service plan objectives for individuals served
- Analyze and prepare concise and accurate documents reports and correspondence
- Effectively market the programs and services of the department
- Train others
- Assist with the general upkeep and cleaning of office areas and agency vehicles
- Exercise sound judgment and critical thinking in decision-making and solving various work-related situations
- Learn and comply with policies procedures and guidelines established by DePaul professional organizations governing agencies and local laws statutes regulations codes and standards related to the area of responsibility
