Position Overview:This position works in the Pediatric ICU providing discharge coordination amid relentlessly high demand. This position is critical to throughput in medical floors. Tasks include family meetings care coordination referrals securing community appointments and collaborating with community agencies programs groups etc. The SW is the liaison between the team and the family for a fast-paced high-stakes sensitive and intense set of scenarios and needs.



Location: Pediatrics at Johns Hopkins Hospital in Baltimore MD



Schedule: Monday - Friday 7:30 AM- 4:00 PM or 8:00 AM - 4:30 PM or 8:30 AM - 5:00 PM

Responsibilities



Assist in coping with the challenges of caring for a child managing their illness hospitalization or outpatient treatment plan
Support families (children parents grandparents siblings) encountering a crisis experiencing grief and bereavement or adjusting to a diagnosis
Help the family and child gain a better understanding of the illness and what to expect while receiving treatment
Discuss illness and/or hospitalization with the child and their family
Advocate for the needs of the child and family with the medical team (doctors nurses therapists) to promote family-centered care
Develop pre- and posthospitalization care plans
Promote a safe family environment
Refer to community agencies for material assistance including temporary lodging or resources needed to support a safe discharge plan
