CLTC Case Manager (Transitions-Orangeburg)

• Provide CLTC case management services to CLTC participants.
• Provide to face to face contact with participant during initial quarterly and annual visits. The case manager will also follow up with participate on a monthly basis.
• Develop and work collaboratively with other providers public and private and division with SCOMH/SCDHHS to provide clients with the most appropriate and effective care.
• Comply with federal state and third-party payor regulations in the delivery of covered services ensuring these are appropriate and that documentation requirements are met as indicated by the SCOMH SCMOMH Corporate Compliment and SCDHHS.

• Bachelor or graduate degree from an accredited college or university in a health or human services field that promote the physical psychosocial and/or vocational wellbeing of the individual being serviced and documentation of at least two years of experience working with target population. The degree must be from an institution that is accredited by a nationally recognized educational accrediting body.

• Bilingual abilities in English and Spanish (or another language) are a plus.

Post hire employees must:

• Have the ability to coordinate interaction between staff document experience skills or training with participation to develop goals.
  
• Possess knowledge of community resources and a working knowledge of families and/or systems theory.
