Senior Doc part of Brigade Health is hiring compassionate Nurse Practitioners (NPs) and Physician Assistants (PAs) to provide high-quality mobile primary care to older adults in the comfort of their homes assisted living and senior communities.

What Youll Do
- Provide mobile primary care visits to seniors
- Manage chronic and acute conditions
- Perform comprehensive assessments & care planning
- Collaborate with caregivers facilities and interdisciplinary teams
- Deliver compassionate patient-centered care
- Maintain accurate EMR documentation
Qualifications
- Active NP or PA license
- 1 years of primary care geriatrics home health hospice SNF or senior care experience preferred
- Strong communication and clinical skills
- Valid drivers license & reliable transportation
