PRIMARY RESPONSIBILITIES

Under the direction of the RN or LPN Preceptor:

• Performs delegated tasks based on demonstrated competence in performing practical nursing care activities for population served. Delegated specialized tasks may include anything for which training was received in the approved practical nursing program.
• Contributes to patient assessments including biophysical psychosocial spiritual cultural and age appropriate needs of the patients including the assessment of nutrition pain management and signs/symptoms of abuse or neglect in order to contribute to the patients plan of care.
• Administers medications and observes patients for adverse reactions to medications and treatments.
• Educates patients/families about disease treatment plan including self-care post discharge holistic needs available resources and follow up care.
• Completes accurate and timely documentation.
• Completes competency requirements satisfactorily.
• Completes training as required; seeks assistance and asks questions as needed to learn and understand role.
• As part of SSM Health nursing department I will demonstrate the professional nursing standards defined in the professional practice model. Uses the ANA Code of Ethics for Nurses to guide his/her response to the current and evolving health and care needs of our patients and our patient populations.
• Works in a constant state of alertness and safe manner.
• Performs other duties as assigned.

EDUCATION

• Graduate of an accredited school of practical nursing
• For Illinois: an officialpassing score on NCLEX results

EXPERIENCE

• No experience required

PHYSICAL REQUIREMENTS

• Constant use of speech to share information through oral communication.
• Constant standing and walking.
• Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
• Frequent sitting reaching and keyboard use/data entry.
• Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
• Frequent use of smell to detect/recognize odors.
• Frequent use of hearing to receive oral communication distinguish body sounds and/or hear alarms malfunctioning machinery etc.
• Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
• Occasional lifting/moving of patients.
• Occasional bending stooping kneeling squatting twisting gripping and repetitive foot/leg and hand/arm movements.
• Occasional driving.
• Rare crawling and running.

REQUIRED PROFESSIONAL LICENSE AND/OR CERTIFICATIONS

• FUTURE - Status of Fail on The National Council Licensure Examination for Licensed Practical Nurses within 3 months of graduation date - Pearson VUE

• And
• FUTURE - Status of Fail on The National Council Licensure Examination for Licensed Practical Nurses within 3 months of graduation date - Pearson VUE

• Future LPN within 6 months
• Or
• Future LPN Compact State
