Make your mark:
- Provide case management and placement support for children and young people in foster care
- Conduct regular visits phone contact and care team meetings
- Prepare reports including court reports case plans quarterly reports end of placement reviews
- Develop and implement care plans aligned with the Looking After Children (LAC) Framework and complete all required LAC documentation
- Identify and respond to risk incidents and child protection concerns in a timely manner
- Provide crisis intervention and support to carers and young people
- Liaise and collaborate with DFFH/Child Protection schools health services and other providers
- Complete Cultural Support Plans

- You get a salary starting at $100776.
- Salary packaging is available up to $15900 plus an entertainment card.
- SCHCADS award level 5.
- Part time 4 days per week or 0.8 which is 30.4 hours.
- Permanent role.
- Work from our great office space in Dandenong.
