Job Summary

Make your mark:

Provide case management and placement support for children and young people in foster care
Conduct regular visits phone contact and care team meetings
Prepare reports including court reports case plans quarterly reports end of placement reviews
Develop and implement care plans aligned with the Looking After Children (LAC) Framework and complete all required LAC documentation
Identify and respond to risk incidents and child protection concerns in a timely manner
Provide crisis intervention and support to carers and young people
Liaise and collaborate with DFFH/Child Protection schools health services and other providers
Complete Cultural Support Plans

Make it work:

You get a salary starting at $100776.
Salary packaging is available up to $15900 plus an entertainment card.
SCHCADS award level 5.
Part time 4 days per week or 0.8 which is 30.4 hours.
Permanent role.
Work from our great office space in Dandenong.
