- JOB SUMMARY
The Outreach & Risk Reduction Navigator works to reduce and prevent adverse health outcome among socially and economically marginalized populations as part of an interdisciplinary team that includes Risk Reduction Communicable Disease and the Coordinated Homeless Response Office (CHRO). This position conducts street outreach to enhance service coordination and facilitate access to care resources and stable housing. This primarily field-based role provides outreach education and navigation services to individuals experiencing homelessness; individuals living with chronic mental illness or substance use disorders; men who have sex with men; members of other sexual minority communities; and others at elevated risk for HIV and sexually transmitted infections.

Minimum Qualifications for Program Specialist 1 (PS1)- 1 year of education or technical training beyond High School; AND
- 1 year of experience related to the area of assignment (i.e. Public Health Community Outreach Health Navigation).
An equivalent combination of education and experience may be accepted. Please note candidates must have at least 6 months of actual work experience to be considered as equivalent.
Special Requirements- Hiring is contingent upon the successful completion of a background check.
- Bilingual in Spanish /English
- Has experience planning and conducting outreach to hard-to-reach populations
- Has lived experience or experience working with individuals experiencing homelessness and other socially and economically marginalized communities; individuals living with chronic mental illness or substance use disorders; communities at elevated risk for HIV and sexually transmitted infections
- Has an understanding of and experience providing trauma-informed services
- Has an understanding of and experience using health literacy principles
