ESSENTIAL DUTIES FUNCTIONS AND RESPONSIBILITIES

  • Responsible for creating recruiting and assembling an Injury Prevention Task Force in collaboration with the Health Promotion Disease Prevention Coordinator Public Health Nursing Director and the Director of Wellness and Fitness to address elder falls.
  • Develop and maintain positive relationships with tribal leadership organizational leadership FDIHB departments community program partners and task force members.
  • Develop defined purpose goals operational logistics (format of meetings voting etc.) objectives scope benchmark and necessary protocols policy communication plan and membership for the Injury Prevention Task Force.
  • Facilitate task force meetings monitor attendance and events observe activities and report results to the Health Promotion Disease Prevention Coordinator.
  • Develop a sustainability plan in conjunction with the Injury Prevention Task Force to ensure the grant program continues after the grant period has ended.
  • Attend and obtain a master certification in an evidence-based practice within one year of the training and facilitate trainings to FDIHB staff and community partners to be taught to community members.
  • Coordinate with FDIHB trained facilitators and program partners to schedule evidence based classes in the community.
  • Travel periodically to communities and other functions to facilitate classes attend tribal injury prevention initiatives and observe program activities as needed.
  • Establish and maintain a folder on FDIHBs share drive to store program information data for evaluation and program results.
  • Work with the Wellness Department to collect summarize and report participant assessment data.
  • Develop a robust marketing campaign in collaboration with the Marketing team to spread prevention awareness and recruit participants for the grant-funded program through printed and social media platforms.
  • Develop and maintain an evaluation plan for project data collection including baseline timeline outcomes and monitor progress periodically.
  • Generate a data collection plan for program evaluation and create necessary tools i.e. surveys.
  • Participate in all required funder conference calls in-person meetings trainings and/or site visits.
  • Responsible for the preparation of the TIPCAP federal grant application and continuation applications.
  • Responsible for submitting required timely progress reports.
  • Comply with the Uniform Administrative Requirements Cost Principles and Audit Requirements for Federal Awards and attend training to ensure ethical compliance for administering the grant.
  • Monitor approve reconcile and review all grant expenditures and ensure costs adhere to the Uniform Administrative Requirements Cost Principles and Audit Requirements for Federal Awards.
  • Confirm with the FDIHB Finance team the grant budgets entered into the accounting system reflects terms outlined in the Notice of Award.
  • Assists the Finance team in preparing for internal and external audits.
  • Oversee the transition of programmatic changes and inform supervisor and funder as necessary.
  • Performs other duties as assigned.

MANDATORY MINIMUM QUALIFICATIONS

Experience: Three (3) years of direct work experience in healthcare and/or prevention.

Education: Bachelors degree in Health Sciences or related field or equivalent experience.

Pursuant to this Policy applicants who meet the minimum qualifications for this position and who are enrolled members of the Navajo Nation will be given primary preference in hiring and employment for this position and members of other federally-recognized Indian tribes will be given secondary preference.
