What Youll Do

• Conduct comprehensive evaluations and develop individualized treatment plans including OASIS paperwork.
• Deliver high-quality evidence-based physical therapy services
• Supervise PTAs as appropriate
• Collaborate with an interdisciplinary team focused on exceptional outcomes
• Build strong relationships
• Contribute to a positive team-driven therapy culture
• Maintain accurate and compliant documentation

What Were Looking For

• Licensed Physical Therapist (PT) in Wisconsin
• Passion for providing home care services
• Strong clinical reasoning and treatment planning skills
• Excellent communication and collaboration abilities
• Energy initiative and a desire to grow professionally
• Commitment to integrity compassion and excellence

• Graduate of an accredited school with a minimum of a bachelors degree in Physical Therapy.
• Certification or license as required by the State of Wisconsin or applicable state to practice as a Physical Therapist.
