Responsibilities
Serves as primary point of contact for referral sources (hospitals physicians outside agencies and other health professionals). Processes all referrals while establishing and maintaining excellent working relationships with referral sources. Receives referrals electronically via fax or telephone. Coordinates with patients/families/caregivers for initiation of care. Collaborates with clinical staff for appropriateness and continuity of care.
Qualifications
Education
High School Diploma High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required
Completion of a course of study consistent with the requirements for Medical Assistants as specified by the Medical Board of California or other equivalent Healthcare certificate program preferred
Experience
1 year of experience in home health intake clinic scheduling or medical office scheduling preferred
