Job Title: Licensed Practical Nurse (LPN) Long-Term Care (LTC)
Location: Milbank SD 57252
Setting: Skilled Nursing Facility / Long-Term Care
Shift: 12-hour Variable Day Shift (6:00 AM 6:30 PM)

Position Summary

Navitas Healthcare LLC is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality direct patient care in a long-term care environment. The LPN will be responsible for medication administration resident assessments clinical documentation and collaboration with the interdisciplinary care team to ensure safe effective and resident-centered care.

Key Responsibilities

• Administer prescribed medications and treatments in accordance with care plans
• Monitor resident health status and promptly report changes to the RN and/or physician
• Perform routine nursing assessments and document findings accurately
• Provide basic nursing care including wound care and other treatments within scope of practice
• Maintain timely and accurate documentation using the electronic health record system (PCC preferred)
• Assist with admissions transfers and discharges of residents
• Support and guide CNAs in delivering resident care as needed
• Ensure adherence to infection control protocols and facility safety standards
• Respond promptly to resident needs concerns and emergency situations
• Collaborate effectively with physicians RNs CNAs and other healthcare team members

Required Qualifications

• Active and unrestricted LPN license (state-specific)
• Prior experience in long-term care or skilled nursing facility preferred
• Ability to work 12-hour shifts including flexibility for scheduling needs
• Strong clinical assessment and documentation skills
• Experience with electronic medical records; PCC system preferred
• Ability to work effectively in a fast-paced team-oriented environment
