About the Role
We are seeking a Physical Therapist for a contract opportunity with Heart of the Rockies Medical in Salida Colorado. This is a strong opportunity for a candidate with outpatient and orthopedic care experience who is looking for a steady rural healthcare clinic assignment in a beautiful Colorado location.
Responsibilities
- Perform comprehensive patient evaluations to assess impairments functional limitations and therapy needs
- Develop individualized treatment plans based on diagnosis prognosis and patient goals
- Provide outpatient and orthopedic physical therapy services within the PT scope of practice
- Design and modify therapeutic interventions to improve mobility strength function and daily living activities
- Support injury prevention wellness fitness and overall patient health education
- Maintain accurate and timely documentation including treatment plans progress notes physician orders attendance and discharge summaries
- Collaborate with healthcare team members to support quality patient outcomes
- Ensure timely completion of therapy documentation and service-related charges
- Requirements
- Minimum 2 years of Physical Therapist experience required
- Outpatient and orthopedic care experience required
- Active Colorado Physical Therapist license required
- Current CPR certification required
- Graduate of an approved physical therapy program
- APTA membership or eligibility preferred
- Ability to work Tuesday Friday 10-hour day shifts
- Strong clinical judgment documentation skills and patient-centered care approach
