Licensed Practical Nurse (LPN) Long-Term Care (LTC)
Location: Milbank South Dakota
Assignment Length: 13 Weeks
Shift: Variable Days & Nights (6:00 AM 6:30 PM / 6:00 PM 6:30 AM)
Schedule: 12-Hour Rotating Shifts
Position Overview
Navitas Healthcare LLC is seeking an experienced Licensed Practical Nurse (LPN) to join a skilled nursing facility in Milbank SD. This role requires strong clinical judgment flexibility with rotating shifts and a compassionate approach to caring for a geriatric population in a long-term care setting.
Key Responsibilities
- Provide direct nursing care to LTC residents in accordance with individualized care plans
- Administer medications and monitor patient responses and outcomes
- Perform routine assessments and accurately document patient conditions
- Assist residents with Activities of Daily Living (ADLs)
- Observe report and escalate changes in patient status to RN/physician
- Maintain accurate and timely documentation in the PCC system
- Ensure compliance with infection control and facility safety protocols
- Promote resident dignity comfort and quality of life
- Collaborate effectively with CNAs and interdisciplinary care teams
- Manage multiple residents in a structured long-term care environment
Qualifications
- Active LPN license (South Dakota or Compact License preferred)
- Graduate of an accredited Practical Nursing program
- Prior experience in Long-Term Care or Skilled Nursing Facility strongly preferred
- Current BLS certification preferred
- Ability to work rotating 12-hour shifts (days and nights)
- Strong clinical documentation and communication skills
- Ability to manage a 1:15 patient ratio effectively
- Compassionate dependable and patient-centered approach to care
- Strong teamwork adaptability and professionalism in a facility setting
