Your new role

We are currently seeking a motivated and enthusiastic Enrolled Endorsed Nurse to join our residential team at our Eunice Seddon Home as a Documentation Assistant on a Full Time this engaging role reporting to the Clinical Supervisor you will be ensuring that relevant documentation for Care Planning is completed and match the current care needs of our residents. This dynamic role is instrumental in ensuring that staff are aware of their documentation responsibilities and that the site is compliant from a documentation perspective.

Your fortnightly roster will be as below:

Week 1 : Monday to Friday (0700 to 1506)

Week 2 : Monday to Friday (0700 to 1506)

Your responsibilities and duties will include:

  • Ensure assessments are completed on a timely basis directly with residents and by liaising with care staff and the Clinical Supervisor
  • Update plans of care as required ensuring the plan of care reflects the residents current care needs
  • Regularly liaise with the care and nursing staff regarding residents whose care needs have changed thus prompting future assessments and documentation
  • Regularly liaise with Clinical Supervisor and Social Support Case Manager on residents assessed needs and planned interventions and support delivery
  • Develop plans of care and act in accordance with the Registered Nurses recommendations.
  • Attend weekly resident planning meetings noting any changes to residents care needs that will require further assessment and potential changes to care needs which may then require the specific residents plans of care to be updated
  • Take a lead role in the use of care planning and assessment technologies as they develop
  • Compare the care needs as assessed to the residents current AN-ACC funding and consider if an An-ACC reassessment is warranted (to be discussed with Clinical Supervisor and or Site Manager)

We are seeking candidates with the following:

Successful applicants will be a Enrolled Endorsed Nurse and have proven ability in undertaking assessment and planning care. You will also possess:

  • Clerical experience and experience in co-ordinating & prioritising tasks
  • Demonstrated experience working in a computerised environment utilising Windows based programs including spreadsheets and word processing
  • Experience working in a related environment
  • Friendly helpful non-judgmental attitude
  • Ability to liaise with staff residents their families and other professionals
  • Ability to work as a team player
  • An understanding of the need for confidentiality and the ability to be discreet with sensitive information
