Position Summary:
The Registered Nurse (RN) in the Health Care Center plays a critical role in providing comprehensive nursing care and clinical oversight to residents requiring short-term rehabilitation long-term care and ongoing medical support. This position is responsible for conducting nursing assessments developing and coordinating individualized care plans supervising clinical support staff and ensuring the delivery of safe effective and resident-centered care. The RN works closely with the interdisciplinary team to promote the health safety dignity and quality of life of each resident.
Essential Duties and Responsibilities:
- Conduct comprehensive nursing assessments to evaluate residents physical emotional and clinical status identifying changes in condition and nursing care needs.
- Assist in developing implementing evaluating and updating individualized care plans in collaboration with residents families physicians and the interdisciplinary team.
- Administer medications treatments and skilled nursing interventions as prescribed by physicians ensuring accurate documentation and adherence to nursing standards and medication administration protocols.
- Provide direct nursing care including wound care catheter care restorative support and other clinical interventions in accordance with the residents plan of care.
- Monitor residents for signs of decline distress infection or medical emergencies and initiate appropriate nursing interventions in a timely manner.
- Communicate effectively with residents families physicians and interdisciplinary team members to ensure continuity and coordination of care.
- Supervise and support Licensed Practical Nurses nursing assistants and other direct care staff to ensure quality care and compliance with established clinical standards.
- Serve as a liaison between residents families and healthcare professionals by addressing concerns providing education and communicating important changes in condition or care needs.
- Coordinate admissions discharges and transfers including required clinical assessments documentation and communication with all necessary parties.
- Ensure accurate timely and complete documentation of nursing assessments care plans medication administration treatments progress notes and significant events.
- Implement and uphold infection control safety and regulatory compliance standards to maintain a safe and supportive environment for residents and staff.
- Participate in care conferences quality improvement efforts surveys audits and other initiatives that support clinical excellence and regulatory compliance.
- Respond promptly and effectively to resident needs emergencies and other urgent clinical situations.
- Other duties as required.
Knowledge Skills and Abilities
- High School Diploma or GED required; Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred.
- Active Registered Nurse license within the state of Kentucky.
- Ability to perform comprehensive nursing assessments and exercise sound clinical judgment.
- Strong knowledge of nursing practice medication administration infection control and resident care planning.
- Ability to supervise delegate and support nursing and direct care staff effectively.
- Ability to provide compassionate resident-centered care while maintaining professionalism and confidentiality.
- Effective verbal and written communication skills.
- Attention to detail and adherence to legal ethical and regulatory standards.
- Willingness to collaborate and work as part of a multidisciplinary team.
- Commitment to ongoing professional development which may include workshops seminars certifications and specialized training related to long-term care rehabilitation or geriatric nursing.
