LTC Community Team Lead Monday-Friday 9a-5p some evenings and weekends We are seeking a dedicated and experienced LTC Community Team Lead to oversee and coordinate our long-term care community operations. The ideal candidate will lead a team focused on delivering exceptional care and support to residents ensuring compliance with regulatory standards and fostering a positive and collaborative environment. Key Responsibilities- Lead mentor and manage the LTC community team to ensure high-quality resident care and operational efficiency.
- Coordinate daily activities and workflows within the community to meet organizational goals and resident needs.
- Ensure compliance with all applicable long-term care regulations policies and procedures.
- Collaborate with healthcare professionals families and external partners to enhance resident care and satisfaction.
- Monitor and evaluate team performance providing feedback and training opportunities to promote professional growth.
- Assist in developing and implementing care plans tailored to individual resident needs.
- Manage conflict resolution and address any concerns raised by residents families or staff promptly and effectively.
- Prepare reports and maintain accurate documentation related to community operations and resident care.
- Support continuous improvement initiatives to enhance service quality and operational processes.
Qualifications- Bachelors degree in Healthcare Administration Nursing Social Work or related field preferred.
- Minimum of 3 years experience in long-term care or community healthcare settings with at least 1 year in a leadership role.
- Strong knowledge of long-term care regulations and best practices.
- Excellent leadership communication and interpersonal skills.
- Ability to manage multiple priorities and work effectively in a fast-paced environment.
- Compassionate and resident-focused approach to care.
- Proficient in Microsoft Office Suite and healthcare management software.
