Shift: Variable Night Shifts
Shift Length: 12 Hours
Guaranteed Hours: 36 Hours/Week

Compensation

• Local Rate: Up to $40/hr (based on experience)
• Travel Rate: Up to $49/hr (based on experience)

Position Overview

Navitas Healthcare LLC is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join a dedicated long-term care and skilled nursing team in Grand Junction Colorado. The ideal candidate will possess strong clinical and patient care skills with experience in long-term care rehabilitation or post-acute settings preferred.

This position requires flexibility for night shifts and the ability to thrive in a fast-paced healthcare environment while delivering exceptional resident-centered care.

Key Responsibilities

• Provide direct nursing care in accordance with individualized care plans and physician orders
• Administer medications and treatments safely and accurately
• Monitor assess and document resident conditions and report any significant changes
• Perform routine nursing procedures including wound care vital signs and basic assessments
• Assist residents with activities of daily living (ADLs) while ensuring dignity comfort and safety
• Collaborate with nurses providers caregivers and interdisciplinary teams to support quality patient outcomes
• Maintain accurate and timely clinical documentation
• Adhere to infection prevention safety and regulatory guidelines
• Support care planning initiatives and resident-focused treatment goals
• Respond promptly and professionally to resident needs during assigned shifts

Qualifications

• Active Licensed Practical Nurse (LPN) license in the state of Colorado required
• Graduate of an accredited Practical Nursing program
• Previous experience in long-term care skilled nursing rehabilitation or post-acute care preferred
• Current BLS/CPR certification preferred
• Strong clinical assessment and documentation skills
• Ability to work independently and collaboratively within a healthcare team
• Excellent communication and interpersonal skills
• Compassionate dependable and patient-focused approach to care
• Ability to manage multiple patient assignments efficiently in a fast-paced environment
