Crisis Navigator - Community Care & Aging (CCA)
In this position you will play a critical role in the Colorado Springs Fire Departments (CSFD) Community and Public Health (CPH) division. You will be responsible for performing assessments intensive case management behavior modification and solution focused treatment for members of our Community Care & Aging program (CCA).
The CCA program provides intensive person-centered navigation services to individuals who are frequent users of emergency services and aging adults who are vulnerable or at risk of losing their independence. The program focuses on reducing reliance on emergency systems improving health outcomes and increasing stability through coordinated medical behavioral health and social service support. Navigators work closely with clients to address complex needs remove barriers to care and promote long-term independence and quality of life.
We will be hiring 2 positions.This is a grant funded position. Current grant funding for this position runs through June of 2029 with continued funding currently being pursued.

Work Schedule:Either Monday-Thursday 8:00AM - 6:30PM or Tuesday-Friday 8:00AM - 6:30PM
JobType:Full-time; Grant-funded; 40 hours/week.
Essential Job Functions the basic job duties an employee must be able to perform:- Conduct outreach and navigation for high utilizers of the emergency response system
- Conduct intensive case management and community navigation for members on a daily basis; manage a caseload of high-needs clients
- Assist and assess individuals referred for physical or behavioral health crises
- Create intervention planning/goal setting for each member assigned
- Complete case conceptualization/presentation in case review internal staffings and community meetings
- Represent CPH at community meetings and report outcomes to supervisor
- Conduct assessments for program enrollment and case management/navigation
- Implement behavioral modification/solution focused treatment for issues that are hindering improvement
- Conduct psychoeducation specific to the clients reported diagnoses and symptomology
- Complete physical and behavioral health intake/assessment for those individuals referred to CPH programs for additional support
- Coordinate care with various community partners including court jail probation attorneys law enforcement primary care behavioral health substance abuse and other available resources
- Participate in internal and external case consultation related to member care access and compliance
- Interact in professional-to-professional communication to advocate for member
- Assist with referral management
- Assist with community outreach building relationships with partners/agencies and maintaining positive alliances
- Monitor emergency medical response interactions and notifications for compliance
We are looking for candidates who demonstrate:
Knowledge of:- Appropriate scope of practice expectations
- Cognitive Behavioral Therapy/Solution focused treatment
- Acute/High needs clientele who have medical and behavioral health challenges
- Substance abuse and polypharmacy concerns/treatments
- Goal setting and treatment planning
Ability to:- Complete assigned tasks with minimal supervision
- Demonstrate leadership and task management skills
- Recognize when to seek additional supervision/consultation
- Identify any ethical concerns that might arise and discuss with supervisor
- Represent CSFD in a professional capacity
- Accept constructive criticism and demonstrate a willingness to learn
- Navigate electronic medical records independently after training
- Set clear and firm boundaries with clients
- Function in a complex and fluid work environment
- Communicate professionally effectively and efficiently with all stakeholders
Minimum Qualifications
- A bachelors degree from an accredited college or university with major coursework in social work counseling psychology or related field
- One year of full-time experience working with patients in a physical or behavioral health setting
- Possess and maintain a valid non-probationary Colorado drivers license not subject to restriction related to alcohol and/or drug violations or pending charges
One (1) year of the appropriate type and level of experience may be substituted for each required year of post-high school education. Additional appropriate education may be substituted for the minimum experience requirements.
Preferred Qualifications
- Experience working with patients in a pre-hospital or crisis setting
Additional Information
Physical Demands: This position requires the ability to lift up to 20 pounds occasionally and 10 pounds frequently.
