Since 1957 weve assisted a wide array of populations throughout Southwestern Pennsylvania focusing on those generally underserved by traditional services including senior citizens veterans individuals with physical and/or intellectual disabilities individuals on the autism spectrum and youths who have aged out of foster care. To further our mission we are looking for aService Coordinator. JOB SUMMARY The Service Coordinator links residents to health and supportive services that will enable the resident to achieve and maintain his/her maximum level of self-sufficiency in independent living. ESSENTIAL FUNCTIONS - Develops and implements a general case management plan for each resident based on the needs assessment.
- Follows-up with residents where appropriate on monthly basis or on the needs of the individual.
- Maintains an active file for each resident containing all information on social service needs.
- Works cooperatively with the property manager to address related issues.
- Handles all reporting responsibilities required by the HUD contract.
- Inspect resident rooms to build rapport identify housekeeping concerns and coach residents on maintaining safe living conditions.
- Assist residents with budgeting and rent management to prevent eviction and promote longterm housing success.
- Make calls and coordinate connections to community resources such as treatment programs benefits case management and crisis supports.
- Maintain daily onsite presence to address concerns promptly deescalate conflicts and reinforce a safe stable environment.
JOB REQUIREMENTS AND EXPERIENCE - High School diploma/equivalent required. Bachelors degree preferred.
- Must be comfortable working with individuals experiencing addiction mental health challenges and hoarding behaviors.
WORK ENVIRONMENT AND PHYSICAL DEMANDS - Physical strength to manipulate light to medium weight materials weighing 10 to 15 pounds on a regular basis.
- Work across multiple program locations including Downtown Pittsburgh the Hill District and McKeesport.
- Use a personal vehicle for travel between sites as needed.
