This innovative facility integrates data and technology into patient assessment care delivery and communication while providing specialized person-centred support for individuals who no longer require hospital-level treatment but needadditionalcare to safely return home or transition back into their community. Reporting to the Manager of Transitional Services the RN is integral in supporting the goal of returning to home/community by being engaged in interprofessional collaboration offering evidence based clinical expertise to inform care plans that fit with patient/family goals using effective communication strategies shared decision-making and mutual support.

About the Opportunity

  • Assesses plans implements directs and evaluates nursing care and functionsin accordance withthe patient-directed approach to care through the established philosophy of service.

  • Performs initial and regular health assessments tomaintainpatient quality of life.

  • Assessing planningimplementingand evaluating nursing care of all patients.

  • Manages patient medicationin accordance withlegislative requirements.

  • Follows the process in place for safekeeping of medications

  • Administers and records prescribed medicationsin accordance withfacility policy.

  • Ensures all completed care tasks are documented appropriately using identified technology and tools.

  • Ensures Continuing Care Assistants/PSWs are completing carein a timely manner.

  • Ensures Continuing Care Assistants/ document all completed care incidents and errors accurately.

  • Applies dressings and treatments according to physicians orders and facility policy.

  • Developmentimplementationand evaluation of care plans.

  • Observes records and reports symptoms reactions treatments and changes in the patients condition.

  • Documents according to facility Patient Care Record Medication Administration Record (MAR) patient progress notesinitiatingmedication incident reports incident reports and other facility specific documentation asrequired.

  • Supports continuous quality improvement riskmanagementand quality assurance activities.

  • Ensures organizational policies are adhered to by team.

  • Ensures patient safety by ensuring team members follow and apply all standard operational procedures safe work practices and training provided.

  • Additionalduties asrequired


About You

In addition to placing high value on continuous improvementcollaborationand accountability you bring:

  • A Bachelor of Science in Nursing and are currently registered with the Provincial Association of Registered Nurses

  • Proof of valid CPR/First Aid Certificationrequired

  • Successful candidates willbe requiredto obtain a clear Criminal Record Checkanda Vulnerable Sector Check

  • Demonstrated supervisory skills and ability to evaluate employee job performance

  • Specialized courses such as: Palliative Care and Venipuncture and Dementia: Understanding the Journey is considered an asset

  • Previousexperience supporting admission and discharge of vulnerable populations with complex social situations; implementing care plans andidentifyingresources that will give the patient every opportunity to be successful in returning to home/community is considered an asset
