The Community Health Worker (CHW) is a trusted member of the community working closely with Shawnee Health medical providers case managers and counseling staff to reach patients and community members. The CHW works with primary care teams and other agencies to improve patient care outcomes close care gaps and assist the patients in navigating the health care and social service systems. This bilingual English/Spanish CHW will work with the Shawnee OB team and provide one-on-one support and health education to pregnant and postpartum parents helping them meet their familys health and wellness goals. The CHW will work with parents throughout pregnancy and up to six months postpartum helping them connect with desired supports in the community access insurance benefits and find a medical home for ongoing care. They will also assist with program administrative duties and community outreach. Essential Job Functions - Engage in the community through outreach to identify and recruit target population high-risk patients from the service area
- Serves as a liaison between Medical healthcare and Social Services and the community
- Provides health education through the utilization of outreach activities informal counseling and social support
- Provides appropriate follow up and feedback to providers on health management plans and goals
- Assists patients/client in utilization of resources including scheduling appointments and transportation
- Assists with Medical Dental and Behavioral health referrals to ensure compliance and follow through
- Assesses the needs of the patient/client and assists in completion of applications for programs/services which they may be eligible
- Provides coaching for patients on effectively managing their chronic health conditions or other health conditions
- Continue to expand knowledge of services and resources available in the area
- Link patients/ clients to appropriate community resources
- Maintain current knowledge of common health issues as well as different types of benefits and insurance
Education & Experience Requirements: - Mandatory
High School Diploma or equivalent - Preferred
