Position: Case Manager- Behavioral Health

Job Summary:
Under the direction of program leadership the case manager provides comprehensive case management services for adults with serious mental illness or families with children who have serious emotional and behavioral issues to improve their quality of life and maintain the highest possible ability to function within the community. This role involves assessing client needs developing individualized service plans linking clients to appropriate resources monitoring progress and advocating on behalf of clients to ensure access to quality care and services.
Primary Duties and Responsibilities
Completes assessments treatment planning to coordinate and monitor services as well as crisis prevention.
- Serves as a liaison for resources and other social service agencies.
- Collaborates with collateral professionals referral sources and providers of ancillary services on behalf of consumers.
- Meet or exceed case load and productivity benchmarks.
- Follows professional and ethical practices when working with patients patient records patients families and community personnel.
- Provide crisis intervention and support as needed.
- Advocate for client rights and promote empowerment and recovery-oriented practices.
- Maintains confidentiality of patient/family information.
- Works effectively with all patients and families to promote a milieu of trust while providing excellent mental health treatment.
- Participates in required trainings.
- Attend all scheduled team meetings.
- Completes all other assignments or duties as designated.
Qualifications
- Bachelors degree in Social Work Psychology or related field.
- Minimum of one year experience in working in a mental health environment with adults.
- Minimum three years experience providing mental health services to families and children or State licensure in Social Work and a minimum of two years experience providing mental health services to families and children.
- Clinical experience knowledge of crisis intervention community resource and solution focused systems approaches.
- Demonstrate competency in computer skills.
- Ability to work in a team.
- Ability to comply with all applicable licensing rules accreditation standards code of ethics insurance reimbursement rules and Judson Centers policies and procedures.
Skills Required
- Strong communication and interpersonal skills.
- Ability to handle sensitive and confidential information.
- Strong understanding of behavioral health diagnoses and treatment modalities.
- Familiarity with community resources entitlement programs and healthcare systems.
- Bilingual abilities may be a plus depending on the community served.
- Strong organizational and time-management skills.
- Ability to work independently and as part of a healthcare team.
- Ability to provide culturally competent services that shows sensitivity to the service populations cultural and socioeconomic characteristics.
Working Conditions
- Clinical office environment; Non-Traditional hours may vary.
- May involve travel to different client homes hospitals or community locations.
- Working with consumers with a variety of behaviors including aggression.
