Summary:
The Intake Triage and Assessment Worker is responsible for deliveringcomprehensive intake psychosocial assessment and triage services across allEleanor Duncan Aboriginal Services programs. The role provides culturallyresponsive assessment risk identification supported decision-making andshort-term case coordination (up to 12 weeks) while facilitating internal andexternal referrals to ensure the most appropriate and timely outcomes forclients. Working within a transdisciplinary model this role collaborates withthe Intake and Triage Nurse and broader program teams to ensure holisticculturally safe and person-centred service allocation.
Responsibilities:
- Conduct culturally responsiveintake psychosocial and risk assessments across all EDAS programs.
- Undertake suicide self-harm DFVand child protection risk assessments and develop safety plans within scope.
- Hold and coordinate a short-termcaseload (up to 12 weeks) while facilitating appropriate referral andallocation.
- Provide supported decision-makingto clients to navigate health social justice and community systems.
- Participate as a decision-maker intransdisciplinary intake referral and allocation meetings.
- Collaborate closely with theIntake & Triage Nurse and program teams to determine urgency and servicepathways.
- Exercise professional judgementand escalate complex or high-risk matters as required.
- Prepare high-quality case notesreferral documentation and risk documentation within required timeframes.
- Advocate for clients withininternal and external service systems.
- Mentor junior intake staff asrequired and contribute to shared practice development.
- Identify systemic service gaps andcontribute to service improvement discussions when requested.
- Maintain strong knowledge ofcommunity government and non-government referral pathways.
- Represent Gulayi in internal casereviews and external interagency meetings when required.
- Uphold trauma-informedstrengths-based and culturally safe practice at all times.
- Maintain accurate data entry andreporting in client management systems.
- Contribute to continuous qualityimprovement processes within the intake function.
Qualifications:
- Bachelor of Social Work or related discipline or significant relevant professionalexperience
- Demonstrated lived experience recognised as a genuine qualification
- Current NSW Drivers Licence
- CurrentNSW Working with Children Check
- SatisfactoryPolice Check no serious/criminal court record
- Demonstrated experience conducting psychosocial and risk assessments
- Ability to undertake suicide DFV and child protection risk assessment and safetyplanning within scope
- Strong communication and interpersonal skills especially with Aboriginal clients andfamilies
- Demonstrated cultural capability and ability to work in a culturally safe and respectfulmanner
- Ability to work autonomously while contributing to transdisciplinary decision-making

- Advocacy andnegotiation skills within healthcare and service systems
- Conflict resolution and trauma-informed engagement
- Understanding of the impact of intergenerational trauma racism and social determinants onAboriginal health
- Awareness of relevant policies frameworks and legislation (e.g. Closing the Gap MentalHealth Act privacy laws)
- Understanding of strengths-based person-centred and culturally safe practice models
- Local knowledge of community-based and mainstream supports housing and employment services
- Engagement with multi-disciplinary teams including LHDs NGOs and primary care
- Experience delivering culturally responsive support and advocacy
