Job Summary
- Primarily responsible for completing assessments medical history review monthly visits and coordinated health care for children with medical complex need who are involved in the child welfare system. Including the oversight of medical appointment medication management provide advocacy for treatment on behalf of the child consultation and education to foster parents birth parents and staff via in person contacts with the persons listed. May work evening and some weekends in order to complete tasks. May travel to high risk communities.
- Proficient in effectively responding to and managing crisis situations providing immediate support and following established crisis management protocols to ensure the safety and well-being of clients or individuals served.
- Actively assist in identifying chronic physical developmental dental and mental health conditions promptly documenting such and providing prompt referral and collateral information as needed.
- Assist LCFS CWS and families in obtaining additional services necessary to educate and support caregivers in meeting the childrens needs.
- Collect health information to determine/facilitate timely linkage to referrals and services needed.
- Work with healthcare professionals and specialists to ensure that children receive appropriate medical treatment and interventions.
- Maintain detailed and accurate health records progress notes and documentation in compliance with agency and regulatory requirements.
- Prepare reports on the health status and care plans for children in the program.
- Conduct Healthy child checkups once a quarter.
- Must spend on average one hour per month with every child in medical specialized foster care contracts within the designated region.
- Must attend well-child coordination (s) meetings (as indicated by supervisor if not hosting)
- Must conduct quarterly review of coordinated care including overall report of coordinated care and any recommendations.
- Collaborate with child welfare teams case workers and legal professionals to advocate for the best interests of childrens health.
- Attend case conferences court hearings and meetings to provide expert recommendations and support.
- Advanced knowledge of pediatric nursing principles healthcare practices and child health assessments.
- Proficiency in delivering health education and guidance to children families and professionals.
- Excellent communication skills both verbal and written for effective interaction with children caregivers healthcare providers and agencies.
- Ability to maintain and manage health records and documentation in compliance with regulations.
- Ability to provide support and healthcare in crisis situations.

Reports possible misconduct or relevant errors/ oversight in a timely manner and responsibly addresses related issues with chain of command.
- Communication/ Interpersonal Skills: Consistently conveys clear accurate timely and respectful communication appropriate for the situation. Informs appropriate management staff when incident occurs or a crisis situation. Maintains professional boundaries with clients/residents staff interns volunteers and vendors. Able to successfully share skills and concepts with others and works effectively with others under time and environmental pressures.
- Safety and Agency Resource Utilization: Demonstrates sound judgement and foresight in safety awareness and in daily work. Notifies in a timely manner the supervisory chain of any incidents or circumstances that need attention. Report all suspicions or allegations of abuse immediately to their supervisor. Ensures appropriate utilization of materials and resources and minimizes waste.
