Job Summary

Position: Intensive Care Coordination with ICCW (ICCW) Facilitator
(Wraparound Facilitator)
Status: Full Time Part-Time/Contingent (Non-Exempt)
Reports to: ICCW Supervisor
Job Summary:
Intensive Care Coordination with Wraparound (ICCW) provides service and support information and referrals for your family based on a plan specifically designed to strengthen how family functioning. The plan is created around the individual needs of your family. ICCW works with your family to identify a support team of people you trust that builds on your familys strengths to help meet your familys needs.
The ICCW Facilitator is a community based position responsible for facilitating family preservation and support services to families eligible for ICCW services. The ICCW Facilitator will assist the family to identify and help with issues such as safety/crisis parenting health/medical emotional social housing education transportation and legal. Families eligible for this program have a 7- 16 years old with a Serious Emotional Disturbance intellectual/development disability or serious mental illness. Services will be provided in the office the clients homes school or other appropriate community outlets.
Primary Duties and Responsibilities
  • Conducting strengths and psychosocial assessments
  • Developing family service plans with the Child and Family Team
  • Providing or arranging outreach crisis intervention & case management services
  • Linking and advocating with other agencies to ensure children and families receive needed health and human services
  • Providing support that enhances or restores the capacity for social functioning in the home school and/or community
  • The ICCW Facilitator is responsible for facilitating the provision of family preservation and support services to families eligible for ICCW services through the Childrens Special Initiatives
  • Monitoring the implementation of the treatment plan creating crisis- support plans with the Child and Family Team when immediate or potential concerns arise
  • Other duties include; attending all court hearings as required acting as an advocate for the youth and family during the court process and enhance the awareness of mental health needs diversity strengths and culture while also providing documentation for all services delivered in accordance with agency standards and guidelines established by funding sources
  • Participate in required trainings.
  • Attend all scheduled team meetings.
  • Perform other duties as assigned.
Qualifications
  • Bachelors degree in human services or related field.
  • Valid Michigan Drivers License and Vehicle Insurance
  • Must be able to complete the required Michigan Department of Health and Human (MDHHS) Services 3-day ICCW Services training for new facilitators in Lansing MI.
  • Must be able to complete 2 MDHHS required ICCW trainings annually.
  • A minimum of one (1) year responsible professional post degree mental health experience specifically with severely emotionally disturbed children and adults preferred but not required
  • Understanding of criminal justice delinquency neglect/abuse issues and organizational and family systems and/or social work practices as it relates to family systems
Working Conditions
  • Job requires non-traditional work hours in a community/home setting.
  • Job requires a flexible schedule.
  • Job includes working with consumers who may show aggression or have behavioral outbursts.
  • Must be able to tolerate varied weather conditions when traveling to clients homes training courses or other community events related to program and agency requirements.
