Job Duties
This position provides social work services and discharge planning for individuals at ESH. Develop and implement person-centered treatment and discharge plans in collaboration with seriously mentally ill consumers and other member of the interdisciplinary treatment team. Serve as link between community agencies and others that support consumers recovery and return to community.
Minimum Qualifications
Minimum of a Bachelors in Social Work (BSW) is required.
Experience providing case management and discharge planning services to the mentally ill population.
Valid drivers license.
Additional Considerations
Experience with community resources for mentally ill population.
