Job Summary
Job Title: Licensed Practical Nurse (LPN) Home Health
Position Summary
The Licensed Practical Nurse (LPN) provides direct handson nursing care to patients in their homes under the supervision of a Registered Nurse (RN) or physician. The LPN supports patient independence safety and comfort while following individualized care plans and agency policies.
Essential Duties and Responsibilities
- Provide skilled nursing care to patients in their homes according to the plan of care
- Monitor and document vital signs symptoms and overall patient condition
- Administer medications and treatments as prescribed
- Perform wound care catheter care ostomy care and dressing changes
- Assist patients with activities of daily living (ADLs) as needed
- Observe and report changes in patient condition to the supervising RN or physician
- Provide education to patients and caregivers on disease management medication use and safety
- Maintain accurate timely clinical documentation in accordance with regulatory requirements
- Follow infection control safety and confidentiality standards
- Collaborate with interdisciplinary team members to ensure coordinated care
- Participate in case conferences inservice training and quality improvement activities
Qualifications
Education and Licensure
- Graduate of an approved Practical Nursing program
- Current LPN license in the state of practice
- CPR certification (BLS)
Experience
- Previous home health or clinical nursing experience preferred
- Experience with geriatric or chronic care patients a plus
Knowledge Skills and Abilities
- Knowledge of basic nursing principles and home health practices
- Ability to work independently and make appropriate clinical decisions
- Strong assessment communication and documentation skills
- Compassionate patientcentered approach to care
- Ability to follow care plans and physician orders accurately
- Basic computer skills for electronic health records
- Reliable transportation and ability to travel to patient homes
Working Conditions
- Work performed in patients homes and community settings
- May require travel flexible scheduling evenings weekends or holidays
- Ability to lift reposition and assist patients as needed

Reports to: Registered Nurse / Clinical Supervisor
