Title: Behavioral Health Care Manager / Complex Care Manager
Location: Ohio (Hybrid Field & Remote)
Coverage Area: Must reside in Northwest Ohio Lucas Fulton Wood or Ottawa County
Duration: 6 Months Contract to Hire
Schedule: Monday Friday 8:00 AM 5:00 PM EST
Travel: 50 60% field-based/member visits Mileage reimbursed
Training Duration: 4 Weeks
Training Format: Virtual Classroom & 1-on-1 (Cameras ON Required)
Pay Range: $35.00 - $44.00 Per hr on w2.
Position Overview
We are seeking an experienced Behavioral Health Care Manager / Complex Care Manager to support care management activities for members with complex physical and behavioral health needs. This role focuses on developing personalized care plans conducting member assessments coordinating services and helping members maintain independence through quality cost-effective healthcare support.
This hybrid role combines field-based member visits with remote documentation and care coordination responsibilities. The ideal candidate will possess strong behavioral health experience case management expertise critical thinking skills and the ability to work independently while managing a flexible schedule.
Position Summary
This role develops assesses and facilitates complex care management activities for members with primarily physical and behavioral health needs. The Care Manager collaborates with interdisciplinary teams providers caregivers and community resources to improve member outcomes and ensure members receive appropriate healthcare services and support.
The role requires strong case management skills behavioral health experience and the ability to independently manage field visits documentation and care coordination responsibilities.
Key Responsibilities
- Develop and implement individualized care plans for members with complex healthcare and behavioral health needs
- Conduct in-person member visits and assessments in the field
- Coordinate care management activities to support quality and cost-effective healthcare outcomes
- Collaborate with healthcare providers behavioral health specialists caregivers and community resources
- Perform ongoing member assessments and monitor progress toward care goals
- Identify barriers to care and coordinate appropriate interventions and support services
- Educate members and families regarding treatment plans healthcare resources and available services
- Maintain accurate and timely documentation within required turnaround times
- Complete all documentation within 24 hours of member visits
- Review member needs urgent requests tasks and care coordination activities daily
- Participate in interdisciplinary team meetings and case discussions
- Ensure compliance with organizational policies healthcare regulations and documentation standards
- Support members in maintaining independence and improving overall quality of life
Typical Day in the Role
- Log into systems by 8:00 AM to review emails voicemails tasks and urgent member needs
- Conduct scheduled member visits in the field
- Coordinate care plans and follow-up activities
- Complete documentation and case notes remotely from home
- Attend meetings and collaborate with internal care teams
- Manage personal field schedule independently
Productivity Expectations
- 5 7 member visits weekly
- Documentation completed within 24 hours
- Timely completion of assigned processes and trainings
Required Qualifications
Education
One of the following:
- Masters Degree in Behavioral Health or Social Work
OR - Degree from an Accredited School of Nursing with 2 4 years of related experience
Required Licensure
Must hold one of the following active Ohio licenses:
