In accordance with the Vision Purpose and Values and strategic direction of the Vancouver Island Health Authority patient safety is a priority and a responsibility shared by everyone at VIHA; as such the requirement to continuously improve quality and safety is inherent in all aspects of this position.

Reporting to the Manager the Nurse Leader Integrated Health Network provides leadership in the day-to-day operations of the multidisciplinary Integrated Health Network team and provides primary health care services including clinical nursing health promotion/prevention/education and chronic disease management by assessing planning delivering and evaluating acute episodic and chronic care needs of patients emphasizing self-efficacy independence and health promotion with a focus on population health quality improvement activities and chronic disease management. Instructs counsels and teaches individual patients and groups. This position coordinates and facilitates the day-to-day activities of the team ensuring effective use of resources and establishment of communication links between other health care professionals management and community services and agencies. Supervises staff provides leadership and functions as a role model and coach to mentor staff with professional development and achievement of identified service goals. Provides leadership within the Integrated Health Network for quality and risk management activities the Chronic Disease Management (CDM) registry development and the use of the provincial CDM toolkit and clinical practice guidelines. Travels to the offices of the family practice physicians participating in the Integrated Health Network.

QUALIFICATIONS:

Education Training And Experience

Graduation from a recognized nursing program or evidence of equivalent combination of education training and experience. Registration with BC College of Nurses and Midwives as a practicing RN registrant. Specialty knowledge and experience relevant to the primary health care practice setting and patient population focus such as Canadian Nurses Association (CNA) certification Advanced Cardiac Life Support (ACLS) Critical Care Level 1 Pediatric Advanced Life Support (PALS); and Five (5) years recent related experience including two (2) years of recent related experience in community health or diverse acute medical/surgical care settings and experience working with seniors and marginalized populations; or an equivalent combination of education and Pulmonary Resuscitation (CPR) certification. Travel is a requirement of this position. Use of personal vehicle and a valid BC divers license is required.

  • Demonstrate knowledge of primary health care principles community development concepts and the strategic direction of the health authority related to primary health care practices.
  • Working knowledge of community resources.
