Sentara Health is hiring for a Behavioral Health Navigator III at Sentara Careplex in Hampton VA!
Status: Full-timepermanent position (40 hours)
Standard working hours: 8am to 5pm EST M-F
Location: Will report daily at Sentara Careplex in Hampton VA!
Overview
The behavioral health navigator level III serves as a senior member of the care coordination team providing expert support for high-acuity behavioral health cases. This role includes workflow leadership mentoring and project participation to enhance service delivery across the continuum.
Education
Masters Degree in one of the following areas: Social Work Counseling Clinical Mental Health Counseling Marriage and Family Therapy Psychology Addiction Studies Nursing or a closely related field ORBachelors of Science in Nursing for RN Candidates
REQUIRED
Certification/Licensure:
Basic Life Support (BLS) - Certification - American Heart Association (AHA) RQI required within 90 days of hire.
Experience
Eight years of healthcare experience delivered in one of the following settings: Acute care long-term care home health hospice skilled nursing facility or community-based program services REQUIRED

Sentara CarePlex Hospital located in Hampton VA is a 224-bed technologically advanced acute care facility and Certified Primary Stroke Center.
