Job Summary:
We are seeking an experienced Registered Nurse (RN) Case Manager to join a dynamic healthcare team in Eureka CA. This role focuses on coordinating patient care ensuring appropriate resource utilization and supporting positive patient outcomes in an acute care setting.
Key Responsibilities:
- Coordinate and manage patient care plans across the continuum of care
- Collaborate with physicians nurses and interdisciplinary teams to ensure effective treatment and discharge planning
- Perform utilization review and ensure appropriate level of care
- Facilitate timely and safe patient discharges
- Communicate with patients and families regarding care plans and available resources
- Maintain accurate and timely documentation in EPIC EMR system
- Monitor patient progress and adjust care plans as needed
Work Schedule & Requirements:
- Night shift schedule (5x8-hour shifts)
- Weekend Requirement: Every other weekend (EOW)
- No on-call requirement
- Floating: May be required between like units based on facility needs
Required Qualifications:
- Active California RN License (required at time of submission)
- Minimum 2 years of acute care experience (required)
- Case Management experience preferred
- Experience with EPIC EMR system
- Strong care coordination communication and organizational skills
