Job Summary

Navitas Healthcare LLC is seeking a compassionate and skilled Physical Therapist with experience in a Skilled Nursing Facility (SNF) setting to join a dedicated rehabilitation team. This role focuses on helping residents achieve maximum functional independence and enhance quality of life through individualized therapy plans and collaborative patient-centered care.

Key Responsibilities

• Evaluate residents and develop individualized treatment plans with measurable short- and long-term goals.
• Provide therapeutic interventions to improve mobility strength balance and functional independence.
• Collaborate with physicians and interdisciplinary teams to ensure coordinated and effective resident care.
• Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes.
• Accurately document evaluations treatments and progress in the electronic medical record (EMR) system.
• Educate residents and caregivers on treatment plans prescribed exercises and use of assistive devices.
• Ensure compliance with Medicare Medicaid and other regulatory documentation requirements.
• Supervise and collaborate with Physical Therapist Assistants (PTAs) as appropriate.
• Participate in care planning meetings discharge planning and quality improvement initiatives.
• Maintain a safe clean and organized therapy environment while adhering to infection control protocols.

Required Qualifications

• Bachelors degree in Physical Therapy (Masters or Doctorate preferred).
• Active Physical Therapist (PT) license in the state of Massachusetts.
• Prior experience in a Skilled Nursing Facility (SNF) setting.
• Strong clinical assessment and patient care skills.
• Knowledge of rehabilitation techniques modalities and assistive devices.
• Ability to work collaboratively within an interdisciplinary team.
• Excellent communication documentation and time management skills.
• Ability to manage a caseload in a fast-paced healthcare environment.
• Proficiency in electronic medical records (EMR) systems.

Preferred Qualifications

• Experience working in high-volume or post-acute care environments.
• Strong commitment to patient-centered care and continuous professional development.
