Job Summary
Navitas Healthcare LLC is seeking a compassionate and skilled Physical Therapist with experience in a Skilled Nursing Facility (SNF) setting to join a dedicated rehabilitation team. This role focuses on helping residents achieve maximum functional independence and enhance quality of life through individualized therapy plans and collaborative patient-centered care.
Key Responsibilities
- Evaluate residents and develop individualized treatment plans with measurable short- and long-term goals.
- Provide therapeutic interventions to improve mobility strength balance and functional independence.
- Collaborate with physicians and interdisciplinary teams to ensure coordinated and effective resident care.
- Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes.
- Accurately document evaluations treatments and progress in the electronic medical record (EMR) system.
- Educate residents and caregivers on treatment plans prescribed exercises and use of assistive devices.
- Ensure compliance with Medicare Medicaid and other regulatory documentation requirements.
- Supervise and collaborate with Physical Therapist Assistants (PTAs) as appropriate.
- Participate in care planning meetings discharge planning and quality improvement initiatives.
- Maintain a safe clean and organized therapy environment while adhering to infection control protocols.
Required Qualifications
- Bachelors degree in Physical Therapy (Masters or Doctorate preferred).
- Active Physical Therapist (PT) license in the state of Massachusetts.
- Prior experience in a Skilled Nursing Facility (SNF) setting.
- Strong clinical assessment and patient care skills.
- Knowledge of rehabilitation techniques modalities and assistive devices.
- Ability to work collaboratively within an interdisciplinary team.
- Excellent communication documentation and time management skills.
- Ability to manage a caseload in a fast-paced healthcare environment.
- Proficiency in electronic medical records (EMR) systems.
Preferred Qualifications
- Experience working in high-volume or post-acute care environments.
- Strong commitment to patient-centered care and continuous professional development.
