Navitas Healthcare LLC is seeking a compassionate and skilled Physical Therapist with experience in a Skilled Nursing Facility (SNF) setting to join a dedicated rehabilitation team. This role focuses on helping residents achieve maximum functional independence and enhance quality of life through individualized therapy plans and collaborative patient-centered care.
Key Responsibilities
Evaluate residents and develop individualized treatment plans with measurable short- and long-term goals.
Provide therapeutic interventions to improve mobility strength balance and functional independence.
Collaborate with physicians and interdisciplinary teams to ensure coordinated and effective resident care.
Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes.
Accurately document evaluations treatments and progress in the electronic medical record (EMR) system.
Educate residents and caregivers on treatment plans prescribed exercises and use of assistive devices.
Ensure compliance with Medicare Medicaid and other regulatory documentation requirements.
Supervise and collaborate with Physical Therapist Assistants (PTAs) as appropriate.
Participate in care planning meetings discharge planning and quality improvement initiatives.
Maintain a safe clean and organized therapy environment while adhering to infection control protocols.
Required Qualifications
Bachelors degree in Physical Therapy (Masters or Doctorate preferred).
Active Physical Therapist (PT) license in the state of Massachusetts.
Prior experience in a Skilled Nursing Facility (SNF) setting.
Strong clinical assessment and patient care skills.
Knowledge of rehabilitation techniques modalities and assistive devices.
Ability to work collaboratively within an interdisciplinary team.
Excellent communication documentation and time management skills.
Ability to manage a caseload in a fast-paced healthcare environment.
Proficiency in electronic medical records (EMR) systems.
Preferred Qualifications
Experience working in high-volume or post-acute care environments.
Strong commitment to patient-centered care and continuous professional development.
