Sentara Health is seeking to hire a qualified individual to join our team as a Licensed Practical Nurse.

Overview:

The Licensed Practical Nurse (LPN) in a home health care setting is responsible for providing direct one-on-one care to patients in their homes. This role involves monitoring patient health administering treatments managing medications and offering essential support to patients with various medical needs. Collaboration with a team of healthcare professionals including Registered Nurses (RNs) and physicians is required to deliver high-quality compassionate care that improves patient well-being.

Anticipated Status/Shift:

Full Time (40 hours/week)

Evening Shift (4:30PM - 12:30AM)

Occasional weekends

Position Location:

Sentara Hospice Services - Suffolk VA (travel within a 40-mile radius)

Pay Range: $22.39 - $37.32/hour

We provide market-competitive compensation packages inclusive of base pay incentives and benefits. Additional compensation may be available for this role such as shift differentials standby/on-call overtime premiums extra shift incentives or bonus opportunities.

Education

• Trade School Graduate for Nursing (Required)

Certification/Licensure

• LPN License in state of practice (VA) (Required)

• BLS (Required within 90 days)

Experience

• 2 years experience (Required)

• Serve as patient advocate in the prevention of illness or disease (Preferred)

• Provide care and treatment for patients and clinical support and teaching for patient and family members (Preferred)

• Customer service testing medication administration and the implementation of nursing interventions and care plans (Preferred)

• Support a holistic approach to patient care (Preferred)
