Who We Are:
Southern NH Medical Centers strives to keep patients on a continuous track to discharge and receive the continuity of care that our patients and their families need. Our Patient and Family Services Department is the driving force to assure communication respect and safety follows our patients throughout the admission phase all the way through the discharge process. This department includes a dedicated team of Case Managers Social Workers and Interpreters that ensure that all care is equal and our patients dignity is maintained above all else.
About the Job:
The Acute Care Case Manager evaluates the patients health status and the patient/family/significant others care capabilities to facilitate the discharge planning process. The case manager develops a multidisciplinary culturally appropriate age-specific discharge plan. They also complete clinical reviews utilizing Internal criteria for the appropriate level of care and insurance coverage.
What Youll Do:
- Assesses and identifies patients needs during the discharge planning process
- Provide resources and community referrals for continuity of care
- Care coordination with families providers and clinical team
- Utilization Reviews
Who You Are:
- Bachelors degree in a healthcare-related field or equivalent experience
- ASN/BSN preferred but not required
- 3 5 years of case management-related health care experience.
- A minimum of 7 years of experience is required without a bachelors degree or a nursing degree.
- Experience with patient documentation in an EMR system preferred

Work Shift:
