Job Summary:
Navitas Healthcare LLC is seeking a highly skilled Outpatient/Clinical Pharmacist to support inpatient outpatient and specialty pharmacy services across a multi-site healthcare system. The pharmacist will be responsible for comprehensive medication management patient counseling and clinical pharmacy services while ensuring compliance with regulatory standards and institutional policies. This role also offers professional growth opportunities including advancement into advanced clinical and leadership pharmacy functions.
Key Responsibilities:
Provide medication management services across neonatal pediatric adult geriatric oncology and OB/GYN populations
Review verify and dispense medications in accordance with clinical guidelines and safety protocols
Provide direct patient counseling on medication usage adherence and adverse effects
Support inpatient and outpatient pharmacy operations across main campus and satellite clinics
Collaborate with physicians nurses and interdisciplinary teams to optimize therapeutic outcomes
Participate in clinical pharmacy programs and specialty care services
Monitor drug therapy effectiveness and provide evidence-based recommendations
Maintain accurate pharmacy records and ensure regulatory compliance
Adhere to hospital JCAHO state and federal pharmacy regulations
Supervise pharmacy technicians and support staff as assigned
Contribute to clinical program development quality initiatives and operational improvements
Support leadership functions within pharmacy services when required
Qualifications:
Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy (BPharm) from an accredited institution
Active unrestricted Pharmacist license in any U.S. state or territory
Minimum 2 years of pharmacist experience required
Minimum 1 year clinical pharmacy experience required
Current AHA BLS certification required
Strong knowledge of pharmacotherapy across multiple specialties
Experience in hospital outpatient or clinical pharmacy settings preferred
Strong communication leadership and patient counseling skills
Ability to work rotational shifts including nights weekends and holidays
Experience working with underserved or diverse populations preferred
Ability to function independently in a high-volume clinical environment
