Work hours:Full-time 8:00am 5:00pm with flexibility requirements with some travel. Occasional evenings and weekends.

Heres What You Need:
Bachelors Degree Social Work Public Health Health Administration or related field of study Required
LBSW - Licensed Bachelor Social Worker Must hold current licensure in the State of Texas (or be eligible for reciprocity if relocating) Upon Hire Required
3 Years Healthcare related experience in social work. Required
Experience in case management services for pregnant women new mothers or to a related population strongly preferred.
Heres What You Will Do:
Care Coordination/Navigation:
Coordinate with NICU Obstetric Emergency Nurse Labor and Delivery Triage nurses social workers care transition manager community health workers to identify patients with post-discharge needs
Assist with ensuring discharge instructions are followed by assisting patient with resources to comply with them. Including but not limited to connecting to follow-up appointments transportation and community referrals
Meet with patients virtually or in person as needed to assist with connection to resources
Assist patients in understanding their care plans and navigating community organization process
Must navigate patients to community resources and ensure connection was made and needs were met.
Conduct non-clinical psychosocial screening to identify social needs safety concerns and barriers affecting Maternal Health patients enrolled in the Maternal Health Continuum of Care Program.
Knowledge of PHQ2 PHQ9 and Edinburgh Screening
Assist in developing and implementing policy and procedures to support individuals experiencing homelessness interpersonal violence and substance use challenges
