Navitas Healthcare LLC is seeking a dedicated Licensed Practical Nurse (LPN) to provide high-quality patient-centered care in a long-term care setting. This role involves direct resident care supervision of nursing assistants and collaboration with interdisciplinary teams to ensure compliance with all regulatory standards and best practices.
Key Responsibilities
Supervise and direct nursing assistants and support staff during assigned shifts
Administer medications and treatments as prescribed
Transcribe and implement physician orders into care plans and documentation systems
Monitor resident conditions and maintain accurate nursing documentation
Perform clinical procedures such as catheterization tube feeding wound care and suctioning
Collect specimens (urine blood sputum) and assist with routine lab tasks
Record vital signs and track patient progress
Coordinate admissions transfers and discharges including required documentation
Communicate effectively with physicians and healthcare team members
Participate in care planning and interdisciplinary collaboration
Ensure availability of supplies medications and equipment
Educate residents and families on care plans and treatments
Maintain HIPAA compliance and resident confidentiality
Assist during facility surveys and inspections
Provide compassionate end-of-life care and support
Required Skills & Experience
Experience in long-term care or skilled nursing Preferred
Strong medication administration and clinical skills Required
Supervisory experience with nursing assistants Required
Excellent communication and documentation skills Required
Ability to prioritize and manage multiple patients effectively
Education Requirements
Graduate of an accredited nursing or LPN program Required
Certifications & Licensure
Active unencumbered Massachusetts LPN License Required
