Position Overview
Navitas Healthcare LLC is seeking a compassionate Occupational Therapist to provide home-based therapy services in St. Johnsbury Vermont. The OT will work with patients in home health and hospice settings to improve independence safety and quality of life. This role requires strong independent clinical judgment and the ability to manage field-based patient care.
Key Responsibilities
Conduct occupational therapy evaluations in patient homes
Develop and implement individualized treatment plans
Assess physical cognitive and environmental limitations
Support activities of daily living (ADLs) to improve patient independence
Educate patients and caregivers on adaptive strategies and home modifications
Collaborate with interdisciplinary teams including nurses and physicians
Document care plans progress notes and visits in HCHB EMR
Monitor patient progress and adjust interventions as needed
Promote fall prevention and home safety strategies
Ensure compliance with home health and hospice regulations
Required Qualifications
Active Vermont Occupational Therapist license
Degree in Occupational Therapy from an accredited program
Valid drivers license and reliable transportation
Ability to work independently in field-based settings
Strong communication and documentation skills
Compassionate approach to home health and hospice care
New graduates accepted (less than 1 year experience required)
