Do you want to help to make every persons discharge from the acute hospital setting and the community go as well as possible to the right place at the right time

We are looking to recruit a Transfer of Care Facilitator to develop and implement trusted assessment processes between hospital and community teams and Care Home providers to improve the process for everyone and reduce delays enabling proactive discharges to care settings where this is required.

This is an exciting opportunity to be working at the forefront with the Health and Social Care system providers of care people and their families to develop and enable a seamless discharge process.



Your role and responsibilities

  • Building supportive relationships with the care market.

  • Monitor the quality and performance of discharges challenging and escalating poor practice where identified.

  • Being proactive and driving opportunities to improve systems and processes.

  • Advocating and negotiating for the best outcomes for people care homes and the health and social care system.

  • Leading development of a trusted assessment process to prevent discharge delays from hospitals.

  • Research learn from implement and reflect upon best practice.

  • Providing insight and feedback on how to develop and change.

You will need

  • a good level of education

  • an understanding of GDPR and patient confidentiality

  • knowledge of adult safeguarding regulation and practice.

The team

The Transfer of Care Hub Team

  • multidisciplinary team of health and social care professionals and services acting as a single point of access which aims to organise coordinate and facilitate safe timely and effective discharge plans following on from their acute hospital stay or supporting admission avoidance

  • the hub is committed to ensuring prompt discharge through early engagement resulting in reduced pressure on acute hospital beds whilst at all times ensuring patients are discharged to a safe and appropriate place

  • the purpose of the Transfer of Care Hub is to ensure that a discharge is managed appropriately and consistently throughout the discharge planning process with all parties clear about their respective roles and responsibilities. Whilst being responsible for providing accurate data to local and national stake holders

  • Transfer of Care Hub operates a 7 day service from 08:00 18:30.
