Pay Range:

$36.00 $43.50 per hour

Shift Information:

Evening Shift: 3:00 PM 11:00 PM
Schedule: 2 days per week every other weekend

Job Description:

We are seeking a dedicated Pennsylvania Licensed Practical Nurse (LPN) to join a Long-Term Care (LTC) team. The ideal candidate will have hands-on experience in LTC settings and be proficient with MatrixCare.

Requirements:

Active Pennsylvania LPN License
Minimum 1 year of LPN experience
LTC experience required
Experience with MatrixCare (required)

Responsibilities:

Provide direct patient care in a long-term care setting
Administer medications and treatments as prescribed
Monitor patient conditions and report changes
Maintain accurate documentation in MatrixCare
Collaborate with nursing staff and healthcare team
