Job Summary
SUMMARY:
NACCs mission is to promote the health & wellness of mind body and spirit of Native American families. We offer a full range of healthcare services that include Medical Behavioral Health Dental and Substance Abuse. The Lead Referral Coordinator oversees the daily operations of the referral coordination team to ensure timely accurate and complete management of patient referrals. This role supports staff workflow monitors referral tracking processes and ensures closed-loop communication between providers specialists and patients. The Lead Referral Coordinator serves as a subject matter expert assists with training and quality improvement initiatives and helps maintain compliance with organizational and regulatory requirements.
DUTIES AND RESPONSIBILITIES:

Team Leadership
Provide day-to-day guidance and support to referral coordination staff.
Assist with onboarding and training of new referral coordinators.
Serve as the first point of escalation for complex referral issues.
Monitor team workloads and assist with task prioritization.
Support manager with scheduling performance feedback and workflow improvements.
Referral Coordination & Tracking
Coordinate outgoing referrals ensuring complete and accurate documentation.
Schedule appointments with specialty providers as needed and communicate details to patients.
Monitor referral status to achieve closed-loop tracking including appointment completion and receipt of consultation notes.
Follow up with external providers to obtain reports and outcomes.
Identify and escalate barriers to referral completion.
Quality & Compliance
Monitor referral tracking metrics and report performance to leadership.
Conduct periodic audits to ensure referrals meet documentation standards.
Support initiatives to improve referral turnaround time and completion rates.
Maintain compliance with HIPAA and organizational privacy standards.
Communication & Collaboration
Act as liaison between providers care teams specialty clinics and patients.
Address patient inquiries related to referral scheduling or status.
Participate in care coordination meetings and process improvement initiatives.

QUALIFICATIONS:

Education:
High school diploma or equivalent required.
Associate degree or certification in healthcare administration medical assisting or related field preferred.
Experience:
Prior experience in healthcare care coordination referrals or prior authorizations required.
Experience working with electronic health records required.
Knowledge Skills and Abilities:
Strong organizational and time-management skills with attention to detail.
Ability to manage multiple tasks and priorities in a fast-paced environment.
Excellent communication and interpersonal skills.
Knowledge of medical terminology and insurance processes preferred.
Proficiency in standard office software and healthcare systems.
