Schedule: Day Shift 5x8 Hours (08:00 AM 04:30 PM)
Hours: 40 hours/week
Position Overview
Navitas Healthcare LLC is seeking an experienced Registered Nurse (RN) Case Manager to support care coordination within a fast-paced acute care environment. This role is responsible for managing patient care plans ensuring appropriate utilization of healthcare services and facilitating safe and effective transitions of care.
Key Responsibilities
- Manage case coordination for an assigned patient caseload (approx. 1:25 ratio)
- Develop implement and evaluate individualized patient care plans
- Coordinate discharge planning and post-acute care services
- Collaborate with physicians nurses and interdisciplinary care teams
- Ensure appropriate utilization of healthcare resources and services
- Maintain accurate and timely clinical documentation
- Communicate effectively with patients and families regarding care plans
- Monitor patient progress and adjust care plans as needed
- Float to affiliated facility as required
- Participate in weekend rotation schedule
Required Qualifications
- Minimum 2 years of acute care case management experience
- Strong background in care coordination and discharge planning
- Ability to manage high-volume caseloads efficiently
- Excellent communication organizational and critical thinking skills
- Prior travel experience required (first-time travelers will not be considered)
Education
- Graduate of an accredited Registered Nursing program
Certifications & Licensure
- Active California RN License (must be active prior to start date; pending accepted)
- BLS Certification (per facility requirements)
