Job Summary
General Summary
Provides coordination of care to patients with select disorders of the GI GU and Integumentary systems including and not limited to abdominal stomas wounds fistulas drains tubes pressure ulcers and incontinence. The Wound Clinic Coordinator provides complex clinical nursing care functions as a resource within the hospital and community a coordinator of care and educator to patients and leadership to staff in the form of coaching mentoring training professional development process evaluation and improvement. This involves structured visits in-service presentations and participation in multidisciplinary meetings when Nursing: Provides first-in-class patient care to a group of patients provides specific assessments according to protocols and procedures patient/caregiver education related to wound ostomy and continence care management. Provides strong collaboration and direction to nursing staff and actively working with physicians nurses and clinical staff to ensure exceptional care.

- Completes required reports (e.g. PI Dash Boards benchmarks etc.) on time.
- Insures that 100% of all employees have current licenses completed yearly health checks CPR orientation or re-orientation where applicable.
- Completes 100% of employee evaluations on time in a thoughtful and constructive manner. For non-bargaining employees follows established guidelines for recommended increases.
- Displays ability and skill in budget development monitoring and remaining within budgetary limits. Flexes costs downward when necessary especially when scheduling and monitoring hours worked.
- Evaluates operations from a workflow perspective and responds appropriately redesigns per customer needs and competitive benchmarks.
- Evaluates patients referred by medical staff nursing staff other disciplines and families; contribute to care planning and provide follow up.
- Completes the nursing process: assessment planning implementing and evaluation on all age groups.
- Correctly identify wounds by type pressure ulcers by stage.
- Follow policy and procedures applicable to individual situations.
- Communicates patient information to appropriate individuals involved in patient care.
- Maintains an organized environment that enhances the role of CWOCN with emphasis on work simplification best use of time and multiple
- role functions. Keeps leaders informed of progress or obstacles impeding goal achievement.
- Keeps current on new legislation and regulations influencing the standards of nursing practice and care and identifies need for proactive changes when applicable.
- Maintains a professional image that reflects the employees role in health care with a positive impression.
- Is involved in professional specialty organization(s) at local state regional and/or national level.
- Reads professional clinical and educational journals and publications to remain abreast of latest developments trends and best practice research.
- Collaborates and participates in data collection of required reports e.g. NDNQI.
- Interprets evaluates and communicates to nursing staff and others research findings pertinent to fields of specialization.
- Participates in the development implementation and evaluation of standards of care practice and policies and procedures.
- Recognizes recurring events or problems which require policy or procedural changes or which signify and educational need.
- Performs as a role model for clinical excellence.
- Provides feedback regarding quality of care to nurse managers directors and/or relevant members of health care team.
- Insures that appropriate personnel are notified to assist employees to achieve performance standards and protect patient safety.
- Develops strategies to enhance revenue and decrease/recover costs.
- Identifies educational need through various means including but not limited to performance improvement activities clinical observations feedback from staff and others.
- Provides clinical expertise to committees that affect patients within specialty.
- Serve as an advisor/resource person upon request to community organizations.
- Demonstrates effective teaching skills through a variety of instructional methodologies. Applies theories of adult learning to educational endeavors.
- Coordinates with Organizational Performance and Effectiveness Department regarding educational programs for staff.
- As directed reviews documentation and billing compliance for the Wound Clinic.
- Performs other related duties as identified.

Qualifications
Qualifications
Minimum Education:
- Diploma Program Nursing Required
- Bachelors Degree Nursing (BSN) Preferred
Work Experience:
- Less than 1 year Experience applying customer service behaviors and communication skills. Required
- Less than 1 year Advanced wound care initiatives with wound clinic experience. Preferred
Licenses:
- Licensed Registered Nurse Upon Hire Required or
- Registered Nurse Multi State License Upon Hire Required and
- Basic Life Support Upon Hire Required and
- Certified in Wound Care within 2 years Required or
- Certified Wound Ostomy & Continence Registered Nurse within 2 years Required or
- Certified Wound Specialist within 2 years Required
Drivers License Statement:
Must possess a valid drivers license in the current state of residence. Driving record must meet requirements established by WellSpan Risk Management.
Courses and Training:
