About the role
As our Care Partner (Registered Nurse) youll support older people across Greater Sydney to live with independence dignity and quality of life. This role has a strong clinical focus working with clients who have complex and evolving needs including those requiring restorative care and end-of-life care.
You will use your clinical expertise to assess needs develop tailored care plans and guide care delivery that aligns with each clients goals preferences and stage of life. Working closely with Community Care Workers and a broader multidisciplinary network youll ensure services are coordinated responsive and aligned with the consumer-directed care model.
Key Responsibilities
Provide phone-based case management and support to clients under the Commonwealth Home Support Programme (CHSP) and Support at Home (SAH) including completing assessments and developing personalised care plans.
Apply a restorative care approach to support clients in maintaining or improving their independence and function.
Use a relationship-focused approach to build rapport over the phone and identify each persons goals needs and strengths.
Build and maintain strong networks with agencies health professionals and community services including ACATs GPs social workers and culturally specific organisations.
Deliver person-centred case management in collaboration with clients families and carers primarily through phone-based engagement.

Skills and Experience
Registered Nurse (AHPRA) with experience in case management within ageing community care or a related setting.
Demonstrated experience working within Support at Home (SAH) and/or the Commonwealth Home Support Programme (CHSP).
Strong ability to deliver person-centred holistic care supporting clients with complex needs including restorative and end-of-life care.
Excellent interpersonal and communication skills with the ability to build trust and engage effectively with people from diverse backgrounds.
Proven ability to work collaboratively within a multidisciplinary environment while managing competing priorities.
Highly organised with strong time management skills and the confidence to represent the service at community events.
Current drivers licence (essential).
