TITLE: Transitional Living Peer Advocate (OP)

JOB SUMMARY
This position is an active non-clinical member of the treatment team for outpatient Transitional Living clients. The person in this position will function with training and life experience as a role model and mentor to TL clients. Direct supervision for this position will be based on the Transitional Living houses they are aligned with and the respective Director.
Qualifications
To perform this job successfully an individual must be able to execute each essential duty satisfactorily in a timely manner. The requirements listed below are representative of the knowledge skill and/or ability required. Ability to maintain open communication with staff and clients; knowledge of regulatory requirements to ensure compliance with standards; and computer application proficiency. Additionally the person in this position must be able to multi-task work independently yet understand elements of the job that require team involvement; possess problem solving skills; and can adhere to strict confidentiality of client information.
Must have dependable transportation.
Education and/or Experience
Minimum of a GED or high school diploma. Must be in the process to be certified as a Peer Support Specialist or already certified. As a person with lived experience the individual in this position will be expected to maintain their recovery via their personal pathway. Training will be provided in the components of Peer Recovery Advocacy. This may include: outreach and engagement; self-help tools; and recovery transitional and pre-crisis and crisis supports.

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Ability to stand reach lift bend knell stoop climb push and pull items weighing 25 pounds or less; manual dexterity; auditory and visual skills; ability to follow written and oral instructions and procedures.
Job Duties
- Provide one-on-one support mentoring and advocacy to individuals in active recovery living in HHI Transitional Living homes and participating in outpatient treatment including completing and explaining the TL agreement to clients assist clients with technology issues as well as letters or case management needs.
- Coordinate with Accounting on DoorLoop Rental Property Management software to ensure client is entered in the system and has one month rent credit TL rent collections; monitor overdue rent report any issues including late fees to Operations Coordinator and Recovery Team.
- Maintain constant communication with Directors Therapists Facilities House Managers Operations Coordinators HHI Finance Department HHI Insurance Team funding sources and referral sources. This is to ensure everyone is aware of issues and needs.
- Ensure clients are completing facility chores for rent credit facility census has current client information verify active insurance monthly verify clients are attending required groups and sessions and document interactions with funding sources and rent or behavioral issues in client note section of chart.
- Provide weekly reports of issues and client progress to the Operations Coordinator the clients clinician funding sources and referral sources. Ensure ROIs are on file.
- Connect clients with support/benefit services and community resources that are relevant to their needs. This may include transportation via HHI vehicles to service medical legal and employment appointments.
- Assist clients with employment readiness navigation facilitate mock interviewing resume development and learning the basics of being a competent employee.
- As a part of recovery planning assist clients with the transition from residential treatment to outpatient treatment and recovery housing. Provide peer support with assimilating into the community and into a life of recovery.
- Collaborate with HHI House Managers in addressing behavioral
- Order supplies for TL houses as needed and
- Act as a motivator for clients new to recovery by fostering engagement and promoting commitment to goals and aspirations.
- Provide peer support during pre-crisis and crisis situations to circumvent relapse situations. Follow all HHI policies and protocols for clinical involvement when a client is in a stressful or tense state.
- Follow all company policies and procedures
- Ensure compliance with state standards and regulatory requirements
- Other duties as assigned
Per insurance employees must be at least 21 years of age and have a valid and drivers license.
