Active State Medical License CDS & DEA (with IA address) Board Certified in ABEM OR ABFM OR ABIM (or osteopathic counterparts) *Must be residency trained in Speciality Certified in* ACLS ATLS & PALS **If ABEM need current ACLS and 1X proof of ATLS** CME past 2 years case logs past 2 years copies of all education & training diplomas SS Card Drivers license color photo emailed to MSO TB/FLU/Hep B/MMR/VARICELLA Immunization records. Must take in person EMR Training prior to first shift- PCC Coordinates. Must have privileges prior to training*** CV DOB Speciaty and red flag must be submitted to MSO to obtain application.

24-hours physician coverage
30-hour NP/PA coverage
Collegial collaboration with HM program also managed by SCP
