Job Summary

Job Summary:

The Registered Nurse provides and coordinates home nursing care according to physicians orders adhering to agency policies and procedures through the application of the nursing process. The RN is responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. This role involves home visits and periodic office meetings requiring reliable transportation a current drivers license and agency-required liability insurance.

Responsibilities:

- Provide skilled nursing care in patients homes.
- Coordinate and direct the provision of home nursing care.
- Follow physicians orders and agency policies.
- Supervise and evaluate Home Health Aides and LPNs/LVNs.
- Conduct patient assessments.
- Perform wound care.
- Manage start of care and follow-up visits.
- Document patient care activities.
- Travel between patient homes.
Required Skills & Certifications:

- Active Registered Nurse license.
- CPR/BLS certification.
- Minimum of 1 year of Adult Home Health experience or 1 year of Hospice experience.
- Proficiency with HomeCare HomeBase (HCHB) EMR system within the last 6 months.
- Oasis proficiency.
- Reliable transportation current drivers license and agency-required liability insurance.
Preferred Skills & Certifications:

- Experience with HomeCare HomeBase (HCHB) EMR system.
Special Considerations:

- Must be willing to travel up to 75 miles from home clinic.
- Mileage is reimbursed at the IRS standard rate from patient home to patient home.
- On-call requirements vary per location and profession.
Scheduling:
- Monday - Friday 8 AM - 5 PM.
- Some positions may have a Baylor Shift schedule: 4x10s Friday-Monday.
- Weekend requirements vary per location and profession.
- 30 points per week productivity expectation.
