Job Summary

Summary: St Francis Hospital is looking for 1 Hematology/Oncology Physician to start as soon as credentialed for up to 4 months.

RATES:

Malpractice Must be included in rates.

Orientation $292.50-307.50 - half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours

Hourly $Per hour not OT

Holiday $877.50-922.50 - Must be 1.5x the hourly rate. Per holiday hour no OT

Schedule/Availability Requirements:

Schedule: M-F 8a-5p- Full time

Start Date: As soon as credentialed

Duration of assignment: 2-4 months

Facility Location: St. Francis Hospital in Federal Way

Setting: Hospital-based outpatient clinics.

Duties: Outpatient clinic setting only no inpatient responsibilities and no call.

EMR: EPIC

Support Staff: Care manager RN MA infusion nurses.

Reason for Coverage: MD departure

Patients/day: Approximately 15-18 patients per day

Types of cases: 60% Oncology 40% Hematology

Credentialing Information:

Timeframe: 60-90 days

Credentialing with other facilities

Billing enrollment - Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less No

Requirement description :

Board Certification BC in Hematology & Oncology Highly Preferred will accept BE

License WA Highly Preferred Will accept IMLCRequired

Fellowship Is Fellowship Required Fellowship Required

Other Certifications DEA Required

Self-Query NPDB report is mandatory at the time of presentation. The Self-Query NPDB report must be dated within 30 days of the presentation date. - REQUIRED
